Objective:

To evaluate the efficacy of the Janus kinase (JAK) inhibitor upadacitinib in a case of highly refractory juvenile idiopathic arthritis (JIA).

Approach:
  • Case Presentation: A 16-year-old female with a 3-year history of polyarticular JIA, previously unresponsive to multiple therapies, was treated with upadacitinib.
Key Findings:
  • The patient achieved marked clinical remission and improved joint mobility after starting upadacitinib.
  • Prior treatments included multiple conventional and biologic DMARDs, including tofacitinib, without success.
Interpretation:

Upadacitinib may serve as a viable salvage therapy for patients with refractory JIA.

Limitations:
  • The study is based on a single case report, limiting generalizability.
  • Long-term safety and efficacy data for upadacitinib in JIA are still limited.
Conclusion:

Upadacitinib demonstrated potent efficacy in a pediatric patient with refractory JIA, suggesting its potential role in managing difficult-to-treat cases.
