Objective:

To present a case of metachronous esophageal squamous cell carcinoma (SCC) developing after treatment for esophageal adenoid cystic carcinoma (ACC).

Key Findings:
  • The patient developed metachronous SCC outside the radiation field after treatment for ACC.
  • Histopathological examination confirmed the SCC with distinct immunohistochemical characteristics.
  • Complete remission was achieved following definitive chemoradiotherapy.
Interpretation:

The case illustrates the occurrence of second primary malignancies post-ACC treatment.

Limitations:
  • The rarity of esophageal ACC limits generalizability.
  • Long-term outcomes and potential late effects of treatment are not fully established.
Conclusion:

Comprehensive endoscopic surveillance is important for early detection of secondary primaries in patients treated for esophageal ACC.
