Objective:

To analyze incidental findings in polytrauma patients undergoing whole-body CT and propose a classification system for their reporting and management.

Approach:
    Key Findings:
    • Incidental findings were detected in 83.9% of patients.
    • Patients with incidental findings had an average of 5.1 findings each.
    • Most findings were located in the abdomen (34.8%), thorax (17.8%), and head (14.0%).
    • 4.9% of patients had findings requiring immediate clarification or treatment (category 5).
    Interpretation:

    The proposed IF-RADS classification system aims to standardize the reporting and management of incidental findings, which are common in trauma CT scans.

    Limitations:
    • Retrospective design may lead to omitted minor findings.
    • Some findings were only detected during secondary review.
    Conclusion:

    The IF-RADS classification can facilitate comparison and clinical management of incidental findings in future studies.
