Objective:

To report the successful treatment of severe plaque psoriasis in a 61-year-old man undergoing peritoneal dialysis, who had a history of ESRD secondary to hypertension and type 2 diabetes mellitus.

Approach:
  • Initial Management: The patient was initially managed with topical corticosteroids and intensified dialysis, which provided transient improvement in psoriatic lesions.
Key Findings:
  • The patient achieved complete clearance of psoriatic lesions (PASI 0) by month 4 of secukinumab treatment.
  • Secukinumab treatment was well tolerated with no adverse events or complications related to peritoneal dialysis.
Interpretation:

This case supports the efficacy and safety of secukinumab for severe psoriasis in patients undergoing peritoneal dialysis, despite the lack of extensive evidence for this population.

Limitations:
  • The study is based on a single case report, limiting generalizability, and there is a lack of extensive evidence for secukinumab use in peritoneal dialysis patients.
Conclusion:

The management of severe psoriasis with secukinumab in this patient suggests a potential therapeutic option for similar cases.
