To report a rare case of pilonidal sinus occurring in the eyebrow region with associated bridging scar formation.
Approach:
Case Presentation: A 56-year-old man presented with a 29-year history of right eyebrow swelling and intermittent drainage following trauma. Surgical excision and reconstruction were performed.

This is the first reported case of pilonidal sinus in the eyebrow region with bridging scar formation.
Pathology confirmed chronic post-traumatic pilonidal sinus with foreign body giant cell reaction.
Surgical excision and staged reconstruction led to significant improvement in scar appearance and visual field at one-year follow-up.

This case highlights the need to consider pilonidal sinus in the differential diagnosis of chronic facial lesions.
