QOL After Head and Neck Reconstruction
A prospective cohort study suggests that clinically meaningful quality‐of‐life changes continue well beyond the first postoperative year, with one‐year complications emerging as the strongest potentially modifiable predictor of later decline.
To evaluate the quality of life (QOL) trajectories in patients undergoing head and neck reconstruction beyond the first postoperative year.
Approach:
Study Design: Prospective follow-up of 315 patients who underwent free flap reconstruction for head and neck disease from 2015 to 2023.
Assessment Method: Patients completed the University of Wisconsin Quality of Life questionnaire preoperatively and at one, two, and three years postoperatively.
Analysis: Quality-of-life trajectories were evaluated using a time-to-deterioration approach, focusing on clinically meaningful declines.
Key Findings:
Quality of life remained dynamic beyond the first postoperative year.
60% of patients experienced clinically meaningful changes between years one and two, and 56% experienced changes between years two and three.
Higher Charlson Comorbidity Index was the only independent factor associated with later quality-of-life deterioration.
One-year complications were identified as the strongest potentially modifiable predictor of future deterioration.

Single-center design may limit generalizability.
Residual confounding could not be excluded.
