Objective:

To report a case of Kounis syndrome potentially triggered by allergic bronchopulmonary aspergillosis (ABPA).

Key Findings:
  • The patient had marked eosinophilia and elevated total IgE during an ABPA relapse.
  • Cardiac troponin I and B-type natriuretic peptide levels indicated myocardial injury.
  • Coronary angiography revealed nonobstructive coronary arteries, supporting the diagnosis of Kounis syndrome.
Interpretation:

This case describes a patient with Kounis syndrome diagnosed during an ABPA relapse.

Limitations:
  • The case is based on a single patient, limiting generalizability.
  • No standardized clinical guidelines for the management of Kounis syndrome exist.
Conclusion:

The findings highlight the presentation of Kounis syndrome in patients with severe allergic airway disease.
