Objective:
To report a case of Kounis syndrome potentially triggered by allergic bronchopulmonary aspergillosis (ABPA).
Key Findings:
- The patient had marked eosinophilia and elevated total IgE during an ABPA relapse.
- Cardiac troponin I and B-type natriuretic peptide levels indicated myocardial injury.
- Coronary angiography revealed nonobstructive coronary arteries, supporting the diagnosis of Kounis syndrome.

This case describes a patient with Kounis syndrome diagnosed during an ABPA relapse.
Limitations:
- The case is based on a single patient, limiting generalizability.
