Objective:

To report a case of a 22-year-old man who developed neuropsychiatric symptoms after chronic use of tiletamine-containing e-cigarettes.

Approach:

    • The patient experienced progressive neuropsychiatric symptoms after chronic use of tiletamine-containing e-cigarettes.
    • Symptoms included gait instability, dysarthria, hand tremor, emotional instability, and marked craving.
    • Brain MRI and electromyography showed no structural abnormalities, while electroencephalography suggested functional brain involvement.
    • Symptoms improved after discontinuation of tiletamine use and treatment.

    Chronic tiletamine abuse via e-cigarettes may lead to persistent neuropsychiatric dysfunction and dependence-related features.

    • The actual chemical composition of the e-cigarette cartridges was not analytically confirmed.
