Objective:

To report a case of long-term control of systemic melanoma with pembrolizumab and successful management of breakthrough brain metastases using stereotactic radiosurgery (SRS).

Approach:
    Key Findings:
    • All intracranial lesions treated with SRS remained locally controlled without sustained progression.
    • The patient maintained excellent functional status (Karnofsky Performance Status 90) without neurological deficits.
    Interpretation:

    Limitations:
    • This is a single-patient observation, limiting generalizability.
    • Detailed molecular and immunologic correlates were not available.
