Objective:

To present two cases of iridoschisis where combined surgical management of cataract extraction and goniosynechialysis was performed and to report the outcomes.

Approach:
  • Case 1: An 81-year-old woman with bilateral iridoschisis and cataract underwent laser peripheral iridotomy followed by combined phacoemulsification and goniosynechialysis. Post-surgery, intraocular pressure (IOP) normalized, and visual improvement was noted over an 18-month follow-up.
  • Case 2: An 82-year-old man with bilateral iridoschisis had standard cataract surgery on one eye. During surgery on the second eye, intraoperative gonioscopy revealed peripheral anterior synechiae, leading to the addition of goniosynechialysis. This resulted in excellent visual outcomes and stable IOP.
