Objective:

To characterize the age-specific heterogeneity in the associations of LDL-C and TG/HDL-C with carotid plaque.

Approach:
  • Participants: Retrospective analysis of 23,058 adults undergoing carotid ultrasonography, with 21,227 included after exclusions.
  • Methods: Multivariate logistic regression evaluated associations of LDL-C and TG/HDL-C with carotid plaque, with age-stratified analyses and interaction tests.
Key Findings:
  • Carotid plaque detection rate was 31.4% (6,657/21,227).
  • LDL-C was independently associated with plaque (OR = 1.12, 95% CI: 1.08–1.17, P < 0.001), strongest in <40 years (OR = 1.51, 95% CI: 1.30–1.76).
  • TG/HDL-C was also independently associated with plaque (OR = 1.11, 95% CI: 1.07–1.15, P < 0.001), stable across age groups.
  • LDL-C correlated with hypoechoic plaques (OR = 1.23, 95% CI: 1.16–1.30), while TyG index did not.
Interpretation:

Limitations:
  • Cross-sectional design limits causal inference.
  • Findings may not be generalizable beyond the studied cohort.
