Objective:

To present a rare case of locally advanced mixed adenoma-large cell neuroendocrine carcinoma of the gallbladder and discuss its management.

Key Findings:
  • The patient was diagnosed with mixed adenoma-large cell neuroendocrine carcinoma with multiple metastases.
  • Postoperative pathology confirmed the diagnosis with negative surgical margins.
  • Immunohistochemistry showed positive markers for neuroendocrine differentiation: Syn (+), CK19 (+), Ki-67 (+) approximately 80%, INSM1 (+), and CgA (+) approximately 10%.
