Objective:

To report a case of asymptomatic right atrial thrombus in a patient with newly diagnosed classical Hodgkin lymphoma and to review relevant literature.

Approach:
  • Patient Case: A 21-year-old woman with classical Hodgkin lymphoma was found to have an asymptomatic right atrial mass during pre-treatment assessment after central venous catheter placement.
  • Imaging Techniques: Staging PET-CT indicated increased FDG uptake in the right pericardiac region. Transthoracic echocardiography identified a multilobulated right atrial mass, and cardiac MRI confirmed its thrombotic nature.
  • Treatment: Anticoagulation therapy was initiated and optimized, enabling the patient to complete chemotherapy without complications.
  • Follow-Up: End-of-treatment PET-CT showed a complete metabolic response, and follow-up echocardiography indicated a reduction in the intracardiac lesion.
Key Findings:
  • Asymptomatic intracardiac thrombosis can occur in patients with hematologic malignancies before treatment.
  • Multimodality imaging is crucial for differentiating between thrombus and neoplastic infiltration in right atrial masses.
  • Anticoagulation therapy can be effectively managed to allow safe completion of chemotherapy.
Interpretation:
