Right Atrial Thrombus in treatment-naive patient with an early-stage classical Hodgkin Lymphoma: A Case Report and review of the literature
To report a case of asymptomatic right atrial thrombus in a patient with newly diagnosed classical Hodgkin lymphoma and to review relevant literature.
Approach:
Patient Case: A 21-year-old woman with classical Hodgkin lymphoma was found to have an asymptomatic right atrial mass during pre-treatment assessment after central venous catheter placement.
Imaging Techniques: Staging PET-CT indicated increased FDG uptake in the right pericardiac region. Transthoracic echocardiography identified a multilobulated right atrial mass, and cardiac MRI confirmed its thrombotic nature.
Treatment: Anticoagulation therapy was initiated and optimized, enabling the patient to complete chemotherapy without complications.
Follow-Up: End-of-treatment PET-CT showed a complete metabolic response, and follow-up echocardiography indicated a reduction in the intracardiac lesion.

Asymptomatic intracardiac thrombosis can occur in patients with hematologic malignancies before treatment.
Multimodality imaging is crucial for differentiating between thrombus and neoplastic infiltration in right atrial masses.
Anticoagulation therapy can be effectively managed to allow safe completion of chemotherapy.
