To report the first case of stage IIIB, ALK-positive lung squamous cell carcinoma (LSCC) achieving complete pathological response to neoadjuvant ensartinib.

The patient achieved a rapid partial response after 4 weeks of ensartinib.
Complete pathological response was confirmed after surgery with no tumor cells detected.
Mild rash was the only observed toxicity during treatment.
