Bilateral Lens Changes Developing Rapidly After Cataract Surgery in a Patient with Severe Myopia

Objective:

To report a unique case of simultaneous bilateral lens abnormalities following cataract surgery in a patient with axial high myopia.

Approach:
  • Case Presentation: A 60-year-old woman with severe myopia underwent cataract surgery in the right eye, which led to rapid development of capsular contraction syndrome (CCS) and lens dislocation in the left eye.
  • Surgical Interventions: The right eye underwent Nd: YAG laser treatment and later surgical anterior capsular release due to significant decentration of the intraocular lens (IOL). The left eye required pars plana vitrectomy and secondary scleral-sutured IOL implantation.
Key Findings:
  • The right eye developed anterior capsular contraction and significant IOL decentration post-surgery.
  • The left eye experienced spontaneous complete lens dislocation into the vitreous cavity without prior trauma.
  • Surgical interventions were necessary for both eyes to address complications.
Limitations:
  • The findings are based on a single case report, limiting generalizability.
  • Long-term outcomes and recurrence of complications were not fully assessed.
