This multicenter observational cohort compared long-term hiatal hernia repair durability after magnetic sphincter augmentation or fundoplication for moderate-to-large hernias.

  • 2

The primary outcome was objective recurrence, defined as more than 2 cm of stomach, fundoplication wrap, or magnetic sphincter augmentation device above the diaphragm.

  • 3

Eligible adults underwent primary laparoscopic repair of an intraoperatively confirmed hernia of at least 3 cm and objective assessment at 3 years or later.

  • 4

The study drew on prospectively maintained registries at six high-volume tertiary referral centers and included procedures performed from 2012 through 2022.

  • 5

Secondary outcomes included GERD-related quality of life, proton pump inhibitor use, patient satisfaction, and revision surgery for recurrent hiatal hernia.
