A 39-year-old woman presented with progressive headache, papilledema, and left homonymous hemianopia due to a large posterior tentorial tumor.

  • 2

    Imaging revealed a 4.9 × 7.1 × 7.0-cm extra-axial mass causing significant mass effect and 7-mm caudal tonsillar descent.

  • 3

The patient underwent preoperative embolization followed by near-total microsurgical resection of the tumor.

  • 4

Post-treatment MRI showed resolution of the syrinx, restoration of tonsillar position, and significant visual recovery.

This case illustrates that acquired Chiari-like malformation and syringomyelia can be secondary to supratentorial lesions.
