This qualitative study explored discharge preparation experiences among 15 grandparent caregivers of preterm infants hospitalized in a tertiary NICU in Jiangsu Province.

  • 2

Interviews conducted on infants’ discharge day identified 3 themes: complex emotions, barriers to intergenerational care and knowledge reconstruction, and a desire for support.

  • 3

Grandparents described feeling marginalized from family involvement while also experiencing happiness about bringing the infant home and worry about the infant’s long-term health.

  • 4

Caregiving challenges included perceived limits of prior experience, feeling overwhelmed by online information, and difficulty assessing specialized care practices for preterm infants.

  • 5

Grandparents wanted greater family support, community resources, and ongoing professional connections, and the authors called for their inclusion in discharge education.
