AI may aid segmental pulmonary embolism detection
Incidental pulmonary embolism detection increased following AI deployment on routine contrast-enhanced CT, with the clearest increase occurring in segmental emboli.
In a single-center retrospective study, incidental pulmonary embolism reporting on eligible non-CT pulmonary angiography examinations increased from 0.35% before AI implementation to 0.63% afterward.
2
The largest increase was in segmental incidental pulmonary embolism detection, which rose from 0.08% to 0.42%; detection at other embolus locations did not change significantly.
3
The AI tool analyzed 4,344 of 5,523 eligible examinations and detected 21 of 32 incidental emboli in processed examinations, for 65.6% sensitivity.
4
Among patients with incidental pulmonary embolism, 77% had an immediate management change, primarily anticoagulation initiation or intensification or inferior vena cava filter placement.
5
The authors called for prospective multicenter studies to assess the prognostic implications and appropriate management of additional incidental emboli detected after AI implementation.
Serial imaging documented 6 years of progressive aortic calcification, culminating in near occlusion before open surgical reconstruction restored blood flow.
