This prospective, multicenter phase 2 trial evaluated denosumab in adults with multiple myeloma and creatinine clearance below 30 mL/min who were ineligible for bisphosphonates.

  • 2

Of 24 enrolled patients, 22 received denosumab; median creatinine clearance was 23.8 mL/min, and 9 patients (41%) were receiving dialysis.

  • 3

    At 3 months, median serum CTX decreased 73% from baseline (n = 18; P = .0002), and 1-year skeletal-related event-free survival was 85.4%.

  • 4

Hypocalcemia occurred in 77.3% of patients, was grade 3 or higher in 31.8%, and required intravenous calcium in 4 patients despite calcium and vitamin D supplementation.

  • 5

The study closed early because of slow accrual and was limited by its small sample size and inclusion of patients with newly diagnosed and relapsed disease.
