This retrospective study compared immune reconstitution after HSCT in children with B-ALL who received chemotherapy alone or immunotherapy before transplantation.

  • 2

Among 312 first-HSCT recipients, 168 received pre-HSCT immunotherapy and 144 received chemotherapy alone; longitudinal immune analyses included 305 patients.

  • 3

T-cell and B-cell counts generally increased after HSCT, but recovery was incomplete: CD4+ T-cell counts and the CD4/CD8 ratio remained below age-expected levels at 24 months.

  • 4

Compared with immunotherapy, chemotherapy was associated with higher levels of several CD4+ T-cell subsets and CD28+ T cells at selected post-HSCT time points.

  • 5

B-cell counts and IgG and IgM concentrations followed similar recovery trajectories between groups, whereas IgA levels and several T-cell subsets differed over time.
