    Burkholderia pseudomallei causes melioidosis, a major cause of morbidity in tropical and subtropical regions. Culture remains the diagnostic gold standard.

    The InBios Active Melioidosis Detect Plus rapid diagnostic test (AMD-RDT) detects B. pseudomallei capsular polysaccharide. Previous studies show good diagnostic accuracy in blood culture broth, sputum, pus, and urine; performance on throat swabs had not been evaluated.

    This prospective study evaluated AMD-RDT accuracy on throat swabs in Cambodia and Laos, using culture as the reference standard.

    Of 1451 throat swab specimens, 38 were culture-positive. Testing of these 38 specimens and 76 culture-negative controls showed AMD-RDT sensitivity of 18.4% and specificity of 100%.

    AMD-RDT positivity occurred only in specimens positive on direct culture and was associated with greater bacterial burden and culture positivity within 48 hours. Its low sensitivity does not support routine use on throat swabs.
