    Rapidly progressive IgA nephropathy (IgAN) and IgA vasculitis nephritis (IgAVN) are associated with severe glomerular inflammation and rapid kidney function decline.

    Two patients with aggressive IgAN and IgAVN received combined complement inhibition and BAFF/APRIL blockade, resulting in reduced proteinuria and stabilized kidney function.

    The first patient, a 74-year-old woman, showed significant improvement in proteinuria and serum creatinine after treatment with eculizumab, iptacopan, and telitacicept.

    The second patient, a 37-year-old man, also demonstrated nephrotic-range proteinuria and improved renal function following similar multi-pathway targeted therapy.

    These cases suggest that multi-pathway targeted therapy may be a promising approach for selected patients with rapidly progressive IgAN or IgAVN.
