This retrospective study compared intention-to-treat survival after surgery or radiotherapy plus androgen deprivation in men with intermediate- or high-risk localized prostate cancer in São Paulo.

  • 2

The cohort included 21,292 men diagnosed from 2014 through 2023: 15,735 initially received surgery and 5,557 received radiotherapy plus androgen deprivation.

  • 3

Before matching, radiotherapy recipients were older and had more high-risk disease and greater comorbidity-related treatment differences than surgery recipients.

  • 4

The primary outcome was cancer-specific survival, with other-cause mortality treated as a competing event; overall survival was the secondary outcome.

  • 5

In the surgery group, 20.2% received at least one additional treatment modality after prostatectomy.
