Retropharyngeal lipoma (RPL) is a rare benign mesenchymal tumor that can grow significantly without symptoms, complicating diagnosis.

  • 2

A 55-year-old male presented with mild dyspnea, dysphagia, and neck swelling, leading to the diagnosis of a giant RPL measuring 140 × 110 mm.

  • 3

Imaging confirmed the diagnosis of RPL, showing a well-circumscribed, non-enhanced fat density mass in the retropharyngeal space.

  • 4

Surgical resection of the giant RPL was performed via transcervical approach with prophylactic tracheostomy, and the tumor weighed 240 g.

  • 5

Postoperative recovery was successful, with normalization of oral intake and resolution of symptoms within two months.
