    Spontaneous ovarian hyperstimulation syndrome (OHSS) is rare in singleton pregnancies and may present with atypical systemic manifestations, including hepatic involvement.

    Two cases of spontaneous OHSS with liver complications were reported, involving a 21-year-old and a 23-year-old woman with abdominal symptoms and elevated liver enzymes.

    Imaging in both cases revealed enlarged ovaries and cholelithiasis, while other causes of liver dysfunction were excluded through comprehensive workups.
