This 10-month prospective cohort enrolled 786 children aged 5–14 years at three tertiary hospitals in Tanzania.

  • 2

Among 744 children with known outcomes, 22 (2.9%) died within 3 months after discharge.

  • 3

Postdischarge death was associated with HIV infection, chronic kidney disease, symptoms lasting more than 2 weeks, leukocytosis, and discharge against medical advice.

  • 4

More than half of deaths occurred within 1 month after discharge, and 13 (59.1%) occurred at home.

  • 5

Pneumonia and malignancies were the leading probable causes of postdischarge death, each accounting for 18.2% of deaths.
