This first-event self-controlled case series used Hong Kong public health records to assess metabolic abnormalities after antipsychotic initiation in patients aged 6 to 17 years.

Among 15,219 antipsychotic initiators, 497 developed metabolic abnormalities: 115 had hyperglycemia and 447 had dyslipidemia.

Compared with baseline periods, metabolic abnormality incidence was elevated on days 8 to 30 after initiation (IRR, 5.60; 95% CI, 2.93-10.70) and peaked on days 31 to 90 (IRR, 7.71; 95% CI, 5.15-11.55

Incidence remained elevated after 90 days of initial exposure (IRR, 3.94; 95% CI, 2.91-5.33) and during subsequent exposure episodes (IRR, 3.71; 95% CI, 2.51-5.49).

The authors cautioned that increased testing near treatment initiation, delayed detection of preexisting abnormalities, and residual time-varying confounding may have influenced the observed patterns.
