    A 74-year-old female with secondary myelofibrosis was initially treated with high-dose ruxolitinib but switched to momelotinib due to transfusion-dependent anemia.

    During treatment with momelotinib, the patient developed significant peripheral neuropathy and arthralgias, which were not managed effectively by dose reductions.

    The patient was subsequently re-switched to low-dose ruxolitinib after improvement of anemia with momelotinib, leading to resolution of neuropathy.

    This case is the first reported instance of ruxolitinib rechallenge following toxicity from momelotinib in a patient with myelofibrosis.
