This population-based cohort included 5,290 Québec participants whose teacher-rated childhood difficulties were linked to hospitalisation and death records from ages 18 to 40 years.

  • 2

By age 40 years, 4.7% had been hospitalised or died from a disease of despair, including drug-related disease in 3.5%, alcohol-related disease in 2.1%, and intentional self-harm in 0.7%.

  • 3

Each 1-SD increase in overall childhood emotional, behavioural, and social problems was associated with a 49% higher adjusted hazard of adult diseases of despair.

  • 4

Disruptive behaviours and ADHD symptoms were associated with higher hazards, whereas prosociality was associated with a lower hazard of adult diseases of despair.

  • 5

Associations were concentrated largely in alcohol- and drug-related diseases, and disruptive-behaviour associations were stronger among females.
