    A 52-year-old man with war-related injuries developed a persistent prosthetic elbow infection caused by multidrug-resistant Klebsiella pneumoniae.

    After 4 weeks of intravenous antibiotics, he received a 14-week multidrug regimen with repeated surgical debridement, but the infection later recurred.

    After conventional antimicrobial therapy failed, personalized phage therapy was initiated with colistin and surgical debridement as an adjunctive treatment.

    The selected lytic phage showed high activity against all 6 K. pneumoniae isolates from the patient, with susceptibility confirmed by agar and liquid-culture testing.

    The combined treatment was well tolerated, with no reported adverse effects, and was associated with progressive wound healing, partial recovery of elbow mobility, and sustained microbiological clearance during available follow-up.
