This post hoc multicenter analysis compares 7 SOFA variants for predicting the composite outcome of all-cause in-hospital mortality and/or intensive care unit (ICU) admission among emergency department (ED) adults with confirmed infection.

  • 2

The analysis includes 2036 adults from the Acutelines, Sepsis 2-3, and SUSPECT cohorts at 2 hospitals in the Netherlands.

  • 3

SOFA scores generally use the worst clinical and laboratory values recorded within 24 hours after ED presentation, regardless of whether patients are in the ED, ward, or ICU.

  • 4

The variants differ in their handling of missing data, adjustment for baseline organ dysfunction, automated calculation, or simplification of the original score.

  • 5

Urine output is excluded where applicable. In the primary analysis, any norepinephrine use receives 4 points because missing pump-dose recordings prevent reliable distinction between low-dose and high-dose administration.
