Pediatric regionalization and limited pediatric readiness leave many emergency departments with little local pediatric expertise.

  • 2

Among 595 injured children, telephone and telemedicine consultation did not differ significantly in primary outcomes, including parental distress and transfer frequency.

  • 3

Telemedicine was associated with better parental ratings of the emergency department discharge or transfer experience.

  • 4

Lower transfer frequency is not necessarily preferable because many transfers may be clinically necessary; safety and outcomes data are needed to assess transfer-prevention strategies.

Further research should address family-centered support before transfer, including the needs of families of the most severely injured children and those facing social needs.
