The study evaluated ICD-10-CM algorithms against repeated eGFR-defined CKD in 8,239,691 adults in the TriNetX US Collaborative Network.

  • 2

Among participants, 1,328,952 met laboratory criteria for CKD stage 3 or higher.

  • 3

For CKD stage 3 or higher, one ICD-10-CM code had 64.1% sensitivity, 95.6% specificity, 73.8% positive predictive value, and 93.3% negative predictive value.

  • 4

Requiring at least 2 codes or restricting code ascertainment to ±3 to ±12 months increased positive predictive value and decreased sensitivity.

  • 5

Temporal restriction raised positive predictive value to approximately 96% for stage 3 or higher and 93% for stage 4 or higher, but no single phenotype was universally optimal.
