Evidence before this study
We did a search of PubMed from Oct 18, 2001, to Oct 18, 2025, with the search terms (“non-small-cell lung cancer”) AND (“tyrosine kinase inhibitor”) AND (“epidermal growth factor receptor”) AND (“combination”) AND (“first-line”). No language restrictions were applied. The search identified two phase 3 clinical trials (the FLAURA2 [] and MARIPOSA trials []) reporting survival outcomes for third-generation epidermal growth-factor receptor-tyrosine-kinase inhibitors (EGFR-TKIs), specifically osimertinib and lazertinib, in combination therapy for first-line treatment of metastatic NSCLC. The FLAURA2 and MARIPOSA trials showed superior progression-free survival with osimertinib-chemotherapy (hazard ratio [HR] 0·62) and the amivantamab-lazertinib combination (hazard ratio 0·70), respectively, supporting the clinical benefit of first-line combination approaches. However, the proportion of L858R mutations was relatively low in both studies (38% in FLAURA2 and 40% in MARIPOSA).
Added value of this study
The AENEAS2 study revealed that aumolertinib combined with chemotherapy significantly improved progression-free survival compared with aumolertinib monotherapy. No new safety signals have emerged with aumolertinib–chemotherapy treatment.
Implications of all the available evidence
Data from the AENEAS2 study supports use of aumolertinib in combination with chemotherapy as a potential alternative first-line treatment option for patients with EGFR-sensitive mutated NSCLC.
